Fatty degenerative osteonecrosis of the jawbone — known as FDOK or NICO — is a condition that remains largely unrecognized in conventional dentistry. These silent, chronic lesions in the jawbone produce inflammatory mediators such as RANTES/CCL5 that can burden the immune system and contribute to a wide range of systemic symptoms.
Diagnosis requires a combination of clinical suspicion, 3D CBCT imaging, and in many cases, measurement of inflammatory markers in the blood. FDOK areas often develop at old extraction sites, particularly where wisdom teeth were previously removed, and may persist for years without causing localized pain.
Surgical treatment involves careful curettage of the affected bone under biological protocols — including ozone disinfection of the surgical site, application of PRF membranes, and supportive supplementation during the healing phase. Many patients who undergo FDOK surgery report significant improvements in chronic fatigue, brain fog, and general immune resilience in the weeks and months following the procedure.
